Your referral may come from your GP or district nurse, from the hospital via your medical team, from a care worker or other healthcare professional. You can even refer yourself or ask a family member to do it for you.

If you would like to refer yourself or have a friend or family member do it for you, please call our Hospice Community Team on 01622 792200.  Alternatively you can ask your GP or healthcare professional to refer you by asking them to download and complete this referral form and email it to THO.ClinicalAdminHoKH@nhs.net or fax it to 01622 718920.

What happens when I’m referred?
When we receive your referral, someone from the Hospice Community Team will call you as soon as possible and often within 24 hours. If you’re unable to receive the call yourself, we will speak to your GP or next of kin.

We will undertake a holistic assessment over the telephone; check your details, medical history, and whether you are safe and comfortable at home. We will check whether you have a package of medical, emotional, spiritual and social care already in place, make referrals to other Hospice, social care or NHS services and advise your GP or healthcare provider on changes to your medical management. When the time is right, we will also discuss with you and your family a plan for your end-of-life care.

Last year our Hospice Community Team managed 1,009 referrals to the Hospice.
