We care for people with a range of terminal illnesses including cancer, neurological disorders and long-term chronic conditions.  We also provide award-winning specialist support for patients with dementia and encourage seeing people early in their diagnosis so we can help them live well and independently for as long as they can.

Most of our referrals come from GPs and hospital and community-based healthcare professionals. Individuals can refer themselves or have a family member or loved one do it for them with their consent.

If you have a patient who you believe would benefit from our care, you can call our Hospice Community Team on 01622 792200 for more advice and information. Or to refer a patient to the Hospice, please download and complete the referral form and email it to THO.ClinicalAdminHoKH@nhs.net or fax the form to 01622 718920.

If the referral is urgent, or you wish to discuss a patient with a member of our team, please call 01622 792200 ext 204.

What happens next?
It’s understandable that when you refer a patient to the Hospice that they and their family may feel anxious and have many questions. Our leaflet A referral to Heart of Kent Hospice should answer some of their questions and help to allay some of their anxiety.

Our community
Every year we care for over 1,800 patients, their families, carers and loved ones. Our care is not just provided within the walls of the Hospice in Aylesford, we also support patients in their own homes and in residential homes.

Out of area
We do take patients from out of the area in certain circumstances. If we do have patients who live outside our catchment area, they are able to access our services as an outpatient or inpatient, but unfortunately we are unable to visit them in their home.
