Advance care planning (ACP) is the term used to describe the conversation between terminally ill patients, their loved ones, and those looking after them about their views and preferences for future care. This is a theory and group work based session.
The workshop helps participants develop the expertise and confidence to begin advance care planning discussions with the people they care for and support them in having those conversations with their loved ones.
