At any one time, we help over 1,000 people living with terminal illnesses to live comfortably in their own homes.

Our Community Healthcare Team are here to support you.

When you are referred to us, you will be contacted by a member of the Hospice Community Team who will undertake a holistic assessment of your palliative care needs. We will review your symptoms, pain and medication and work closely with your GP and other healthcare professionals to ensure you and your family are receiving the care and support you need. 

We can offer practical assistance in the form of advising on equipment that could help you maintain your mobility and independence around the home. We can also provide emotional & spiritual care, welfare advice, social support, as well as guidance and support to parents of young children. Sometimes with your permission we will offer to refer you for additional support from other NHS or Charitable services that we have assessed may be of benefit to you. We can also support you and your loved ones with conversations and guidance about planning for your needs when you are less well or maybe approaching the end of your life.  

You can contact our Hospice Community Team by telephone on 01622 792200. We offer a 7 day a week service between the hours of 8:30am to 5:30pm. Out of hours enquires are automatically diverted to colleagues on our Inpatient Unit.

Hospice Hub services

Our Hospice Community Team encourage you and your loved ones to come to the Hospice and access our specialist compassionate care and support via our new Hospice Hub service. 

Our Hospice Hub offers a range of sessions where our patients, family members and carers can access our specialist care, support and advice, as well as the opportunity to share peer-to-peer support with others facing similar challenges.

Amanda, from the Hospice Community Team, calls me regularly, every two weeks and always asks how I am, ensuring I have the right support and care. Every contact I have had with the Hospice has been excellent. Everyone there does their utmost to understand you and to support and give you help where needed.

The Hospice made such a difference. Joerg and the community team helped me more than they will ever know. With their help, Peter was fighting to the end. When he died, he knew I was with him. He had his children around him and his ‘mutleys’ - our two Westies - lying across his legs.

Compassionate Neighbours

Are experiencing loneliness or social isolation? A Compassionate Neighbour volunteer could help you.

Our Compassionate Neighbours project matches local volunteers with those under our care who may be feeling lonely or isolated. You will have the opportunity to build a genuine friendship with your Compassionate Neighbour volunteer, who will visit and call you regularly.
