Our community team stood in our garden at the hospice

We offer specialised care for people aged 18 and over who are living with a terminal illness. We also support their families, carers and friends.

Most of our referrals come from GPs and hospital and community-based healthcare professionals. Individuals can refer themselves or have a family member or loved one do it for them with their consent.

Every patient is unique, so we tailor our care and support to their individual needs, taking a holistic approach that considers their physical, emotional, spiritual and social wellbeing.

We will be in touch

One of our team will contact the patient as soon as possible to discuss your needs and the support we can offer.

We’ll ask about your illness, symptoms and current care.

We will get to know you

Following the initial call, we may arrange a more detailed assessment, either at the hospice or in your home.

This assessment helps us understand your needs and work with you to identify the best support for your concerns.

We will work with you to ensure you get the support you need, from the right person, in the right place, at the right time.

We understand that being referred to a hospice can feel daunting, and you may have questions about what happens next.

Working alongside the professionals involved in your care, our team will do everything they can to make your experience the best it can be, always treating you, your family and friends with respect, compassion and dignity.

Once you have completed your referral, we will help you to work out which of our services can best support you, and put you in touch with the relevant teams.

Our teams are committed to giving our patients and their loved ones flexibility, control and independence, supporting them wherever they feel most comfortable and happy.


Hospice Community Team


Our Hospice Community Team can visit you at home or in your care home, or welcome you as an outpatient at the hospice, helping you manage symptoms and supporting those closest to you.

Inpatient Unit (IPU)


Our Inpatient Unit offers calm, welcoming care for symptom management or end-of-life support, with admissions from home, a care home or hospital, and stays tailored to individual needs.

Family Support Team (FST)


FST are here to support you, and those closest to you, in the weeks and months ahead, offering emotional, spiritual and practical support, including help with finances and benefits.

The Hospice Hub Team


Our Hospice Hub offers a welcoming space where patients, families and carers can access specialist care, support and advice, and connect with others facing similar challenges.
