This Dying Matters Week, Heart of Kent Hospice paramedic Mickey Hill explains how an Advance Care Plan can bring peace of mind - for you and the people you love.

"Thinking about what might happen if you became too unwell to speak for yourself can feel daunting and many people naturally try to avoid those thoughts. Yet taking gentle, thoughtful steps to consider your wishes can be an act of care for both you and the people who matter to you.

"One of the most helpful ways to do this is through an Advance Care Plan. At the hospice, we support individuals in writing down the kind of care they would want, whether that's the treatments they'd feel comfortable with or the place they'd hope to be cared for if they could no longer manage at home.

"As a paramedic, I have seen first-hand the difference having an Advance Care Plan in place can make. Having your wishes recorded means your loved ones won't have to face difficult questions or make assumptions on your behalf. It also provides health care professionals attending your home with a clear idea of what your wishes would be, should you be too poorly to communicate your preferences.

"These conversations don't have to happen alone. Talking openly with those close to you can bring clarity and reassurance for everyone involved. When your loved ones understand what matters to you, they can honour your choices with confidence and love.

"It's important to remember that an Advance Care Plan is not permanent. Your feelings and preferences may change over time and you can update your plan whenever you need to. Once it's written you can simply keep it safe, knowing it's there to help guide others when the time comes.
